Give always context to comment reply / thread

Sometimes it is not giving context or the hierarchy of discussion thread is mixed up with no clear chronology. Mainly when I come from notifications.

Example: this comes from a comment a made to Horacio and I have no context.

In this one Hierarchy should be:

• My comment first because is the main one

• Bellow the answers by chronology.

In this one I have no context of where this reply comes from, I should see my comment first which Eric is replying too.

On this one "Ok let me review..."Should be just after Eric's comment and then the comment box at the bottom, because if not I lose the chronology of replies.
